Mail Order Instructions for Dakota Growers Pasta

  1. Please click on the Mail Order Form.
  2. Print out the form.
  3. Fill in the Bill To information with the address of the person paying for the product.
  4. Fill in the Ship To address if you would like the product shipped to a location different than the Bill To address in step 3.
  5. The right side of the form contains a column called Quantity. Please use this area to identify the case quantity of each item you would like to purchase. Leave the lines blank for the items you do not want. There are two prices listed for each item. Find your state in the list on the left hand side of the page. Use the pricing zone indicated to calculate your total cost.
  6. Fill in the Total Case Qty and Total Price lines on the bottom left side of the form.
  7. Place the Mail Order Form in an envelope along with a check or money order, made out to Dakota Growers Pasta Company for the specified amount on the form, and mail to:
